School Year 2025-2026 Registration

The 2025-2026 School Year begins Monday, August 18th, 2025 and goes through Thursday, June 4th, 2026.

Mandatory File Paperwork 

*All file paperwork must be turned in at least one week prior to your child's first day of school. Your child will not be allowed to enter the classroom without ALL file work completed. This is a DCFS requirement.

​

Please download each of the following documents and submit them for your child's file.

 

• Application (all 3 pages)

• Emergency Information Card

• Child Medical Form:

  • Examinations must be done annually

  • All medical forms must be signed by the doctor

  • The medical form on this website must be used

  • Medical forms must include the following:

    • TB test with result

    • Lead screening with result

    • Vaccination history

    • Varicella (chicken pox) vaccine

    • Health history (yes/no questions to be completed and signed by parent)

• Hearing and vision screening are required annually for children ages 3 and up 

• Copy of child's birth certificate

• DCFS Licensing Standards (sign and return last page)

• Guidance & Discipline Policy (sign and return last page)
